Below are links to the forms needed to schedule your sleep study with Comprehensive Sleep Medicine. Please fax these forms with the required documents to us when they are complete, and once we have received a doctor's referral, we will contact you to schedule your sleep study.

Preparation for your In-Lab Sleep Study

What to bring:
- Toiletries (toothbrush and paste, contacts, hairbrush, etc.).
- Sleeping clothes are mandatory, and should not be loose fitting and comfortable, no silky materials. Two-piece pajamas are ideal.
- Medications normally taken before bed (technicians may not dispense medications).
- Change of clothes for the morning.
- Insurance cards (for our records only).
- Any paperwork completed ahead of time (see links to forms below).
- Optionally, a pillow, white noise machines, a fan (all rooms are equipped with a ceiling fan), reading material, and/or a laptop computer to use before bed (each room has wireless access and flat-screen cable TVs), may be brought to the sleep study.
Please do not bring valuables or a lot of cash, cigarettes, cigars or pipes. Smoking is not permitted at the sleep center.
How to prepare for your sleep study:
- Shower before the appointment (avoid moisturizes or conditioners on your hair and skin).
- Please refrain from smoking, caffeine, and stimulants at least 24 hours before your appointment.
- Notify staff if you have a tape allergy.
- Cancellations should be made at least 1 business day before your appointment.
- The alarm is set at 11p.m. and turned off at 6a.m. Notify a technician if you must exit the center between these times.
- We respectfully request you be ready to leave the sleep center 45 minutes after awakening. The center closes at 7a.m.
Electrodes must be placed on your scalp. Toupes or weaves MUST be removed for your appointment.
